Backstory
"One of the few true pacifists in this dangerous world. Indeed, the Doctor is very skilled at 'pacifying' people. With chloroform. But business is currently booming, what with all the beating being administered by Cops since the Mayor's new "Law of the Land" was put in place. Despite the extra cash flow, the Doctor would rather the streets be safe, and has decided to join the Resistance."

The Doctor is an NPC and playable character. They are a starting character. They have a unique ability, Chloroform Hankie, that can instantly knockout unaware NPCs from behind. They wear a lab coat, dress shirt, and tie, along with a head mirror - stereotypical doctor garb. They are the only playable character that starts with the Pacifist trait.

Super Special Ability[ | ]

NPC[ | ]

Doctors are most often found in Hospitals, standing still or moving from one appliance to another; Hospitals can be recognized by their distinct checkerboard floor tile pattern. Doctors never carry weapons of any sort.* Despite being pacifists, Doctors will attack with their fists if provoked.

When interacted with, players can:

  • Pay $60 (on 1-1, cost increases linearly per floor) to be fully healed.
  • Have the Doctor Administer a Blood Bag, sacrificing 20 HP and giving you a Blood Bag item.
    • Donating blood can only be done three times in total per floor, regardless of whether it is done for a Blood Bag or Money. For example, if a player donates blood to a Clerk two times, they have 1 more possible blood donation available on that floor, which may be used to create a Blood Bag.
  • Use a Blood Bag to restore 20 HP.

Doctors are unable to draw their own blood and require other doctors to create Blood Bags.

On Bounty disaster floors, Doctors who are bounty hunters don't use weapons against the player and instead attack with their Fists; however, they still carry and drop weapons such as Axes and Shotguns.

If a Doctor administers a Blood Bag for you, they will vote for you.

Tips and Strategies[ | ]

  • How to fight: Doctors have a unique playstyle, as they are Pacifists and cannot use most damage dealing items. This often forces Doctors to play as a stealth characters, as not doing so can end a run very quickly. Use your Chloroform Hankie to instantly neutralize people from behind and the Tranquilizer Gun when necessary. Hiring party members and giving them strong weapons is also a good way to handle combat.
    • While the Pacifist trait can be swapped or removed at an augmentation booth, doing so is expensive ($300 or $600 respectively), counterproductive towards the Doctor's Big Quest, and less effective due to the Doctor's low Melee and Firearms stats. Doctors will usually have plenty of spare cash, however, so it might be helpful to remove the trait so you can fight in emergencies.
    • Even with Pacifist, you can still attack with your Fists. Considering your Melee stat is 1 however, only use it in desperate situations as you won't survive against bat-wielding Goons or other strong NPCs.
    • Chloroform Hankie should be used as little as possible in front of NPCs, as they will become annoyed, then hostile upon seeing someone chloroformed.
    • Chloroform Hankie can be used on annoyed individuals without sneaking up on them by simply walking inside them and right clicking until they're in the right position.
    • NPCs behind a wall can be chloroformed by hitting a wall near them and running behind them while they are turned around.
    • Try to get a Taser as it can paralyze an enemy allowing you to Chloroform them easily. Just make sure NOT to do it in front of allies as they'll become hostile as well.
    • The knockout delay on the Tranquilizer Gun can be used to your advantage against groups of enemies. Because it's silent, the target will not be alerted if the player is not in their line of sight, and can be knocked out unnoticed as long as the player leaves in time; as aligned foes who see the target fall will be hostile for a short time, so don't get caught.
    • To knock out two characters simultaneously, shoot one of them unnoticed and knock out the other with Chloroform. If timed right, nobody will notice and you will still be eligible for the 'No one alerted' bonus (if for a mission).
    • Using Killer Thrower (Consumable) is still allowed, allowing for "non-violent" insta-kills.
    • Doctors may use EMP Grenades against Cop Bots to kill them without violating their oath.
    • Having hired Gangsters or Soldiers kill for the Doctor does not violate their oath either, with Medical Professional allowing Players to bring their follower back to full health once per floor.
    • Leafblowers also allow killing without penalty by knocking enemies into holes, fire, and other such hazards.


  • How to finish missions: Avoid direct conflict when possible, and use the tranquilizer only when needed to keep ammo costs low.
    • If low on cash and ammo, consider bribing mission targets if a mission cannot be completed without starting a fight.
      • Gangsters may also be hired, but are usually more costly and less reliable than just using the tranquilizer gun.
  • If struggling, take the time to Chloroform Drug Dealers, Slum Dwellers, and anyone else you can knock out for free XP and other items. The Doctor can knock out an entire floor without fighting if careful.
  • Choose traits with no relation to melee or gun weapons. For example, Team Building Expert goes well with Medical Professional. However players can still stockpile weapons for allies to use. Know their class strengths and weaknesses: are they good at melee? Hand them a bat! If your minions have a good weapon already there's no need to give them anything.
  • If a player prefers to go it alone, selling weapons such as bats and guns to the Sell-O-Matic can land a decent profit.

Trivia[ | ]

  • The Doctor once started with an item called Body Vanishing Salt.
  • The Doctor's "Pacifist" trait is a reference to the Hippocratic Oath, which states that medical professionals are to "Do no harm" under penalty of being stripped of their medical license.
